$5 Sports Physicals Clinic
Corewell Health Helen DeVos Children’s Hospital & Baxter Wholistic Health Center is pleased to offer $5 sports physicals for grades 6-12 on Tuesday, November 12, 2024, from 6-8PM here at Baxter Community Center’s Wholistic Health Center. A parent or guardian must be present to receive a sports physical.
These forms are mandatory for service & will available during the event:
Michigan High School Athletic Association (MHSAA) physical exam form & Corewell Health Consent Form signed by a parent or legal guardian
**Exams will not be completed without the required materials.**
Walk-ins are welcome, but pre-registration is encouraged.
